Chronic conditions

56 y/o Caucasian male presents to the primary care clinic with complaints of dizziness and nausea x 4 days. The patient reports he has not been able to get out of bed since the symptoms started. The patient reports symptoms are worse when he tries to get out of bed to stand. He denies any headaches or blurry vision. He states he is urinating more over the last few days and he has noticed an increase in thirst. He reports he just drank a large sweet tea before he came into the clinic.

The patient reports that he is out of his Lantus and metformin because he cannot afford the refill until he gets his disability check. He is disabled after his second CVA that left him with generalized weakness. His medical history includes DM, HTN, CAD.

Upon arrival at the clinic, the patient’s vital signs are as follows- Blood sugar 405, B/P 190/101, HR 102, R-20, T- 98.5.

The chair of the Network’s ad hoc task force

You are the chair of the Network’s ad hoc task force. Your charge is to evaluate the two alternatives and to make a recommendation on which one to accept, if revenues would be identical for the two alternatives, and hence the decision can be made solely on the basis of costs. As part of the analysis, the costs of the two alternatives must be estimated on a per procedure basis and an annual basis. In addition, any relevant qualitative factors must be considered before the recommendation is made.

To keep the base case analysis manageable, the task force was instructed to assume that the operating costs remain constant over the useful life of the equipment. For comparative purposes, this assumption is not too egregious because the activities are roughly the same for both alternatives and, hence, inflation would have a somewhat neutral impact on the cost comparison.

In addition, the System CFO has asked the task force to perform some sensitivity (scenario) analyses. He is concerned about the accuracy of the cost detail inputs. Although he is confident about many of the estimates, he thinks others are more arbitrary. Those activity cost inputs considered to be most uncertain are supplies cost per unit; billing and collection cost per unit; general administration cost per unit; and transportation, setup, and breakdown cost per unit.

Thus, first, the task force must redo the analysis assuming that these inputs are higher than the base case values by 10 and 20 percent. Activity cost inputs less than the base case values could also be examined, but the critical issue here is not to underestimate the total costs involved in the two alternatives. Second, the task force must determine what would happen to the cost estimates if the useful life of the capital equipment were as short as three years or as long as seven years. Another concern was that the useful life of the equipment depended on the alternative chosen; that is, there would be less wear and tear under Alternative 1 than under Alternative 2. Finally, the task force must assess the impact of a purchase discount: Would the discount amount influence the ultimate decision?

Five Reasons the US Entered WWI

1.Economic Interests-The American businessmen were very interested in the Allied victory and many such as J.P. Morgan helped fund British and French war efforts with approximately $3 billion in loans and bond purchases. If the Allies would be defeated by the Central Powers, they probably wouldn’t be able to repay their debt to their US lenders. Many businessmen therefore supported the so-called ‘Preparedness Movement’ which campaigned for the US intervention in the war on the side of the Allied forces.
2.Sinking of the Lusitania-In May 1915, a German U-boat sunk the British passenger ship Lusitania off the coast of Ireland. Over 1,000 passengers were killed, including 128 Americans. Although the ship may have been carrying military equipment along with the civilians, the Americans were infuriated because the people on board weren’t warned before the sinking. In addition to straining diplomatic relations between the US and Germany, the Sinking of the Lusitania further increased anti-German sentiment in America.
3.Unrestricted Submarine Warfare-In response to Britain’s blockade, Germany turned to unrestricted submarine warfare to keep goods from reaching Britain. After the Sinking of the Lusitania, Germany promised to stop unrestricted submarine warfare but within less than one year, they torpedoed another passenger ship – the cross-English Channel ferry Sussex. Again, the Germans promised not to attack passenger ships without warning (the Sussex Pledge). But that pledge was short lived as well.
4.Zimmermann Telegram-In 1917, German Foreign Minister Arthur Zimmermann sent a telegram to Mexico suggesting that if the US should declare war on Germany, Mexico should declare war on the US In return, Mexico would get back the territory lost in the Mexican-American War (Texas, New Mexico, and Arizona). Unfortunately for Germany, the telegram was intercepted by the British and hurriedly given to the Americans. Although Mexico had no real intention of declaring war on the US, the publication of the letter further mobilized the American people against the Central Powers.
5.German Atrocities in Belgium-One factor that had a major influence on American public opinion was the invasion of neutral Belgium and stories of German atrocities in the country which shocked and outraged the Americans. Stories of unarmed civilians being killed and small towns being destroyed circulated throughout the press. Although some of the stories were British propaganda, they left a strong anti-German sentiment among Americans.
